To ensure we’re the right fit for your care, we ask that all prospective patients complete our New Patient Intake Form before scheduling an appointment.

Once your form is submitted, a member of our team will reach out to coordinate your visit and guide you through the next steps.

New Patient Appointment Times

Appointments for new patients are available:
• Tuesdays & Thursdays at 6 PM
• Wednesdays & Fridays at 1 PM

You’ll be greeted by our friendly staff and asked to sign a few quick forms before beginning your personalized consultation. We’re here to support you every step of the way as you begin your journey to optimal wellness.

Payment & Rescheduling
To confirm your appointment, full payment is required at the time of scheduling. If you need to reschedule, we kindly request at least 24 hours' notice. Your payment will be applied to your first visit.

Lab Work & Imaging Reports
If you have any lab work, X-rays, MRI, or CT scan reports, please email them in advance to reception@nmccenters.com, or arrive 15 minutes early to have them scanned before your consultation. Early submission helps ensure Dr. Kendra has time to thoroughly review your information.
